I know it seem like a lot of information but it really helps me to know a bit more about you and your cat(s) before I get to actually work hands-on with them.
There are three parts to this form:
• Part 1 is your general information
• Part 2 (Cat #1) is specific information about your cat.
• Parts 3 & 4 are the same as Part 2 if you have additional cats that you want me to work with.
